Your Health Care Directive informs hospitals what kind of medical treatment you want or don't want, and authorizes someone you trust to make your medical decisions.

Remember to also fill out a HIPAA Release form so your health care agent can obtain necessary information about your medical condition from your health providers. You should be able to find one online. Please note that your HIPAA must be notarized to be legally effective.

Your Power of Attorney authorizes someone you trust to make financial decisions for you if you’re unable to do so. 

For example, you may critically ill in a hospital and you need your spouse to access your bank accounts to pay the bills. Without a Power of Attorney, your spouse would need to go to probate court and persuade the judge to give her access. This could take months and thousands of dollars in attorney's fees. If you have a power of attorney, your spouse would automatically have the power to make your financial decisions and access your bank account.

Your Last Will & Testament ("Will") informs your family who can receive your assets so your house and money don't go to the wrong people. 

If you own a house or other properties, I encourage you to consider a Living Trust (instead of a Will) so you can avoid the tens of thousands of dollars in probate fees and have more control over how your family can receive your assets.
